Requires a person who files a fetal death certificate to also prepare a certificate of birth resulting in stillbirth. Requires that the certificate be filed as in the case of a certificate of live birth and be offered to the parent or parents of the stillborn child. Provides that a parent of a stillborn child who was born before the effective date of this amendatory Act of the 93rd General Assembly may apply for a fetal death certificate. Illinois SB 1649 (93rd) — VITAL RECORDS-STILLBIRTH
Amends the Vital Records Act. Requires the State Registrar to prescribe and distribute a form for a certificate of birth resulting in stillbirth.
