Illinois SB 2695 (94th) — NURSNG HOME-TRANSFER/DISCHARGE
Amends the Nursing Home Care Act. Provides that pending approval from the resident, a nursing home must inform the resident's designated case coordination unit of the resident's pending discharge and must provide the resident or his or her representative with the case coordination unit's telephone number and other contact information.
