Illinois HB 4003 (97th) — MHDD CD-MANDATED FORMS
Amends the Mental Health and Developmental Disabilities Code. Repeals provisions requiring the Department of Human Services to prescribe all forms necessary for proceedings concerning (i) the admittance of a person as an inpatient to a mental health facility for treatment of mental illness and (ii) the admittance of a person with a developmental disability to a facility for residential and habilitation services.
