Illinois HB 5775 (99th) — BIRTH CERT-PARENTAGE FORMS
Amends the Vital Records Act. Provides that voluntary acknowledgment of paternity, denial of parentage, and rescission of voluntary acknowledgment of paternity or denial of parentage forms shall contain the data elements required by federal law.
