Bill Support For Transition From Institutional Settings Joint Budget Committee. The bill directs the department of health care policy and financing (department) to provide community transition services and supports to persons who are in an institutional setting, who are eligible for medicaid, and who desire to transition to a home- or community-based setting (eligible persons). Colorado HB 18-1326 (2018A) — Support For Transition From Institutional Settings

Joint Budget Committee. The bill directs the department of health care policy and financing (department) to provide community transition services and supports to persons who are in an institutional setting, who are eligible for medicaid, and who desire to transition to a home- or community-based setting (eligible persons).
